Rice County Health Assessment Survey
Local health officials seek your help in assessing the health of our community.
Rice County Public Health, District One Hospital, Northfield Hospital & Clinics and HealthFinders are collaborating to assess the health of Rice County. Over 1,000 randomly selected households in Rice County will receive a health survey in the mail next week. Local health and wellness providers will use the information gathered to help understand and improve the health of our community.
Completing the survey is voluntary, although everyone’s participation is important. The confidential survey will ask adults questions about their health status, lifestyle habits such as nutrition, exercise, smoking, and alcohol use, access to health and dental care, and health insurance coverage. In addition, opinions will be asked about perceptions of health related issues in Rice County. The survey is part of a collaborative community health assessment process, to assist in identifying areas of greatest community health need.
By completing this survey, you will be helping local health providers better understand how we can work together to improve the health of Rice County residents,” said Deb Purfeerst, RN, Director of Rice County Public Health. “The information we gather from this survey will provide direction for future health initiatives and programs.”
