Long Term Care

 

Information for the Public

Rice County Public Health is the "lead" agency for Alternative Care, Elderly Waiver, Community Alternatives for Disabled Individuals (CADI), Community Alternatives for Care (CAC), and the Traumatic Brain Injury (TBI) waiver. As such, the Agency is responsible for implementation of Long Term Care Consultation (formerly referred to as Pre-Admission Screening) activities and for administering waivered service programs designed to assist ill and disabled individuals to live at home or in other community-based settings.

To help achieve this goal, Rice County contracts with board and lodge, adult foster care, assisted living, adult day services, meals-on-wheels, respite, home care, and related providers. Also, with an increasing number of disabled persons under the age of sixty-five living in community rather than institutional settings, Rice County Public Health has sought additional vendors able to provide services such as pre-vocational training and supported employment. Rice County is fortunate to have a wide variety of long-term care resources for its residents.

Information for Contracted Vendors

Throughout 2009, Rice County Public Health, in partnership with the Faribault Senior Center, will offer educational programs of interest to our contracted vendors, at no cost to the attendees.  Information about the programs will be posted on this site, and contractors are encouraged to take advantage of these opportunities for staff and owner education offered here in Rice County at no charge.
